    Copier Refund Form

     Refunds will be given after account activity verification.  

    We apologize for your inconvenience.

     

    Instructions:

    After completing form select the SUBMIT button. Your form will be sent to the Auxiliary Services department. Upon verification, your account will be credited within three (3) business days.

    Campus

    ID# *Number printed on your AccessCard/ID or V-Card. NOT your Social Security Number.

    Name

    Number of copies

    Copier *Located on front of copier.

    Date of Loss *If not current date.

    Email (REQUIRED)   *Please enter your email address in case we need additional information.

    ***Refunds will not given for copies made in error.***

    ***You are responsible for adding money to your Card***
