Technology and Media Forms

Media Request Form

Our Media Request form is designed to facilitate equipment requests, such as TV/VCRs, laptops, LCD projectors, CD players, slide projectors and screens. This form should be submitted by your director or department chair at least twenty-four hours before your equipment is needed.

Technology Work Order form

Our Technology Work Order Form is designed to facilitate your technology needs. Any requests you have requiring service or repair of any of the equipment the Technology and Media department maintains should be submitted through this form. All work orders are addressed in the order they are received.

Dunwoody Graphics Form

Our Dunwoody Graphics Form is designed to facilitate requests to the Dunwoody Graphics Center. Any requests you have involving creation and/or printing of any media through the Dunwoody Graphics Center should be submitted using this form. Requests made to the Dunwoody Graphics Center should be submitted at least two weeks prior to the project deadline

AxisTV Form

Our AxisTV form is designed to facilitate requests for the new Dunwoody Campus News Network (Our AxisTV plasma news system). Any information you would like to distribute through the AxisTV system should be submitted using this form.
